MLPB’s 7th Annual Conference: Register Online at http://bucme.org/live/1476/fees

TREATING SOCIAL DETERMINANTS OF HEALTH: CRITICAL SKILLS FOR THE HEALTH WORKFORCE

Social, economic, and environmental conditions always have been key drivers of poor health outcomes and harmful health disparities. What’s new? There is greater recognition by the healthcare system that improved outcomes, quality, and cost management are tied to effective detection, triage, and treatment of social determinants of health (SDOH).

With full CME/CNE accreditation opportunities (and Certificates of Attendance available for Community Health Workers), you’ll learn from expert faculty about:

  • How best to support patients facing housing instability and homelessness
  • Role-appropriate strategies to help patients overcome barriers to accessing health-promoting entitlements like SNAP and disability benefits
  • Best practices for facilitating patient/caregiver access to medical transportation and FMLA protections
  • "Reality-checking" on SDOH that lack solutions under current public policy
  • And other topics related to this emerging competency!

You’ll also hear from leaders on key policy issues that will impact how SDOH are accounted for in successful person-centered care systems – such as risk stratificationrisk adjustment, and SDOH screening protocols.
